If the following statements apply to your situation, please read the following guidance fully and carefully.

| If this is the case it's not a problem, ask the business who have told you that you need a disclosure to contact us directly. Or - if you are self-employed and have others in your organisation who can check your identity (not related) please contact us using our pack request form. Individuals cannot apply for their own Disclosure

| I have been asked by my employer / prospective employer to have a check carried out. |
